The Aware Counselling Service Referral Form

For GPs, NGOs, and other healthcare professionals

This page is intended for GPs, NGOs, and other healthcare professionals wishing to refer a client. If you are looking to self-refer, please fill in your detail’s here.

GPs, NGO’s and other healthcare practitioners may use the form below to refer individuals who are seeking support for mild to moderate difficulties.

Please advise your client that we will contact them directly once we have processed the form. Submitting a referral does not guarantee acceptance. Please manage client/customer expectations. Screening outcomes are usually shared within 10 working days.

Our programme is aimed at individuals, who reside in Ireland, who are experiencing mild to moderate depression, stress and/or anxiety and would like to learn new ways to deal with life’s challenges.

If your client can self-refer and opt in, please direct them to input their details here.

If your client is unable to self-refer (e.g. due to language or a disability), GPs, NGOs and health professionals can complete our referral form on their behalf.

Please include relevant clinical information, such as risk factors or mental health history.

If a client is seeking long-term therapy or presents with complex mental health needs, we would encourage you to signpost them to another counselling service or support service. The Aware Counselling service is a short-term intervention designed to support people with mild to moderate difficulties.

The Aware Counselling Service is not best placed to meet:

  • Active and acute risk to self and/or others
  • History of psychosis
  • Currently experiencing significant emotional or personal crises, who may not feel socially or emotionally ready to engage in regular psychological therapy
  • A diagnosis of a personality disorder, schizophrenia, or other psychotic conditions, whose needs may be better supported by more specialised services
  • Drug or alcohol dependency
  • Seeking treatment for Autism or ADHD

We screen each referral to assess suitability. If we are not the best fit for your client’s current needs, we will signpost to an alternative service within Aware or externally.

We also accept referrals from NGOs and community organisations using the referral form. The same screening process applies.
